The Reds were left to rue another penalty loss against Arsenal after Harry Wilson missed the all-important spot kick.
Liverpool faced Arsenal for the second time in four days, making nine changes from their 3-1 win at Anfield on Monday as James Milner looked flat but Neco Williams was superb.
Adrian replaced Alisson in goal, with the Williams duo and James Milner coming in to the back four as Kostas Tsimikas was unavailable due to injury.
Wilson made his first appearance for the club since 2017, with Marko Grujic and Curtis Jones also coming into the side.
Mo Salah retained his place, but was joined by Takimu Minamino and Diogo Jota as Sadio Mane and Roberto Firmino watched the game from home.
The first half offered little – except one chance at the end of the 45 as Arsenal goalkeeper Bernd Leno parried Jota’s header into the path of Minamino, who could only hit the bar with his effort.
Arsenal’s best opportunity came when the ball fell to Eddie Nketiah one-on-one with Adrian – but the Spaniard made himself big to palm the strike away.
The second half was more of the same, with Jota again testing Leno and Virgil van Dijk being denied by the German.
After the score went to 4-4 after five, Wilson missed and Joe Willock converted, sending Arsenal into the quarter-finals.
